What Are Festoons?

Festoons, also known as malar bags, are genetic conditions that present themselves as puffy bags around the lower eyelids and above your cheeks. These under-eye bags can result in a tired, older-looking appearance. Festoons may appear as swollen circles beneath the eyes and may look similar to dark circles under your eyes.

Operation: To address her concerns, this patient underwent correction of congenital festoons, removal of fat attached to the undersurface of the orbicularis muscle of the lower eyelids, midface lift, orbicularis muscle suspension and canthopexy.

Diagnosing The Causes Of Festoons / Malar Bags

Dr. Asaadi pioneered the “Botox® Test” to properly diagnose the causes of your festoons or malar bags. There are two main causes of festoons: excess fat and muscle laxity. It is important to first diagnose the correct cause in order to develop a proper treatment plan. During the Botox® Test, a tiny amount of Botox® is injected into the orbicularis oculi (the area around your eye). If your malar bags get worse after the injection, then the root cause of your festoons is likely muscle laxity. However, if you experience no change after the Botox® injection, then the presence of excess fat is likely the cause of your malar mounds.

What Is Festoon Surgery?

During festoon surgery, Dr. Asaadi will address the cause of your malar bags or festoons. If excess fat pockets are contributing to your festoons, then Dr. Asaadi can surgically remove these fat deposits underneath the skin. If your malar mounds are caused by lax muscles, then Dr. Asaadi can release the ligaments of the surrounding eye muscles to reduce puffiness associated with festoons. Part of the muscle can also be removed, if necessary, while the remaining muscle can be attached to a stable area below your eyelid. Dr. Asaadi will discuss which method of festoon treatment is ideal for you.

What Can I Expect Before My Festoon Treatment?

You may be a candidate for malar bag treatment if you have puffy bags underneath your eyes and above your cheekbones. Dr. Asaadi will carefully examine your festoons and administer the Botox® Test to diagnose the cause of your malar bags. He will discuss a proper treatment plan with you during your consultation for festoons.

Festoon surgery patients will receive specialized pre-care instructions beforehand. You may need to avoid certain foods, drinks, supplements, or medications in preparation for your malar bag surgery. We encourage you to plan for a few days of recovery immediately following your festoon surgery. You should also arrange for a friend or family member to bring you to and from surgery.

What Can I Expect After My Malar Bags Treatment?

Patients may experience slight bruising, swelling, or tightness after malar mound surgery. Eye drops and cold compresses can be provided to ease discomfort and assist healing. Stitches are typically removed after 1 week.

Patients are advised to avoid exercise and strenuous activities for a few weeks following your festoon surgery. You may also be advised to limit reading, television, and your usage of computers and tablets for a few days to reduce strain and allow for proper rest. Your festoon recovery will ultimately vary depending on the specifics of your procedure.
